Radiological Physics Associates, Inc. (“RPA,” “we,” “us,” or “our”) is a medical physics consulting firm based in Nashville, Tennessee. Information you give us
When you submit a service request, contact us by email or phone, or otherwise communicate with us, we may collect:
- Contact details for you and any other contacts you list, such as name, job title, email address, and direct or department phone numbers.
- Organization details, such as facility name, phone number, shipping and billing addresses, client status, accreditation types, state facility registration numbers, and radioactive materials license expiration dates.
- Equipment and project details, such as modality, room or location, equipment make and model, inspection and installation dates, and shielding construction information.
- Billing preferences, such as preferred payment method, whether a purchase order is required, and where reports and state forms should be sent. We do not collect credit card or bank account numbers through the Site.
- Anything else you choose to include in a message or description field.
Information collected automatically
Like most websites, our hosting provider automatically records basic technical information when you visit, such as your IP address, browser type, device type, pages visited, and the date and time of your visit. This information is used to operate, secure, and troubleshoot the Site.
We also use a tracking script from Insightly, our customer relationship management platform. It may set cookies and record pages you visit on the Site. If you have contacted us or received an email from us, this activity may be linked to your contact record, which helps us understand which information is useful to our clients and follow up on requests.

Patient and Health Information
Please do not submit patient information or protected health information (PHI) through the Site. Our online forms are designed to collect business, facility, and equipment information only. How We Share Information
We do not sell or rent your personal information. We share information only as needed to run our business:

- Regulators and accrediting bodies, when you ask us to submit forms or documentation on your behalf, or when required by law.
- Legal and safety purposes, when we believe disclosure is required by law, subpoena, or court order, or is needed to protect the rights, property, or safety of RPA, our clients, or others.

Cookies
The Site uses a small number of cookies that are necessary for it to function and stay secure. We keep information for as long as needed to provide our services, maintain business and regulatory records, resolve disputes, and meet legal, accounting, and accreditation requirements.
